I certify all the information submitted on this application is true and complete and that I have read and understand the "Application and Acceptance Policy & Procedure" and the "Student Policies". I understand the discovery of false, misleading, omitted or misrepresented information may cause the rejection of my application or my termination from Mechanics Local 701 Training Fund programs.

I expressly authorize the Mechanics' Local 701 Training Fund, its representatives, employees or agents to contact and obtain information from all references (personal and professional), employers, public agencies, licensing authorities and educational institutions and to otherwise verify the accuracy of all written and verbal information provided by me in the application process.

I hereby waive any and all rights and claims I may have regarding the employer, its agents, employees or representatives, for seeking, gathering and using truthful and non-defamatory, in a lawful manner, in the employment process and all other persons, corporations or organizations for furnishing such information about me.

I understand my application remains active for 1 year and, at the conclusion of such time, a new admission application and payment of application processing fee will be required. In consideration of my acceptance in the training program, I agree to conform to the current published policies and procedures of the Mechanics Local 701 Training Fund and any future revisions and/or addendums that may occur during my enrollment if I am accepted into the program.

I understand that as the enrollment start date approached, I will be contacted on details of when I assigned interview date and time is scheduled. Any required documentation that must be presented for consideration will need to be submitted at that time in original forms, a drug test with a passing result will be required, and any other indicated testing/processes. This cannot be rescheduled. If I cannot make the assigned interview process date, I must notify the training fund at the time I am notified of assignment, and my application will be considered for the following start date. Any no shows to the interview will have to complete the entire application process and resubmit an application processing fee. 

I also understand that if accepted and I terminate my scheduled enrollment at anytime, or miss the orientation/start date, I may no longer be eligible for the program.
